This Privacy Policy defines the principles of processing personal data by the UNI-DENT Stomatology Center s.c., with its registered office at ul. Wolumen 18, 01-912 Warsaw, tel. 503-009-729, e-mail: centrum@unident.pl, in accordance with applicable law, including Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR).

  • Fulfillment of legal obligations – in order to fulfill obligations arising from legal regulations, such as maintaining medical records, in accordance with art. 6 sec. 1 letter c of the GDPR,
  • Direct marketing – in order to send information about our services, with your consent, in accordance with art. 6 sec. 1 letter a of the GDPR.

4. Scope of personal data processed
We only process data necessary to achieve the above purposes, such as:

  • Name and surname,
  • Contact details (address, telephone number, e-mail),
  • PESEL and ID number,
  • Information about health to the extent necessary to provide medical services.

5. Data recipients
Your data may be transferred to:

  • Entities cooperating with us to the extent necessary to provide services (e.g. laboratories, IT suppliers),
  • Bodies authorized to receive them under the law (e.g. National Health Fund, Social Insurance Institution),
  • Companies providing accounting, legal and IT services based on data processing entrustment agreements.

6. Data storage period
We store personal data for the period resulting from the law, in particular for the mandatory period for storing medical records (20 years). Data processed on the basis of consent will be stored until the consent is withdrawn.

You have the right to:

  • Access your data and receive a copy of it,
  • Rectify (correct) your data,
  • Delete your data, limit its processing or object to its processing – in cases provided for by law,
  • Withdraw consent to data processing (to the extent that the data is processed on the basis of consent),
  • File a complaint with the President of the Personal Data Protection Office.

We provide appropriate technical and organizational measures to protect your personal data against unauthorized access, loss or destruction.
